TRANSMITTAL FORMS

NDX has several forms to assist you in the indexing of your files and boxes. If you know the content of your boxes, you can index on just the box level. You can also index on the file level, which is a complete list of each file or item within each box. This method is the most organized way to manage your records.

Please contact NDX to request file and/or box transmittal forms. We will email the form(s) to you in Excel format. When finished entering your data, save and email to request@ndxarchives.com.
